Background: The Covid-19 pandemic in Indonesia increased in August 2021. The government has made various efforts to control Covid-19, one of which is the provision of Covid-19 vaccination with elderly as the main target. Survey conducted by the Indonesian Ministry of Health in 34 provinces in September 2020 showed 64.8% of the elderly accepted, 27.6% were in doubt, even 7.6% rejected Covid-19 vaccination. The initial study in Ngijorejo in 2021 showed from the total of 95 elderly,70 people participated in Covid-19 vaccination while 25 others didn’t. Objective: This study aimed to know elderly motivation level to participate Covid-19 vaccination in 2021 in Ngijorejo Gari Wonosari Gunungkidul. Method: This was a quantitative descriptive study using questionnaires. The sample was 40 people selected by purposive sampling technique. Pearson Product Moment was employed with 15 valid points. The reliability test used Alpha Cronbach's formula of 0.931 therefore the instrument was reliable. Result: This study showed elderly motivation level to participate Covid-19 vaccination in Ngijorejo Gari Wonosari Gunungkidul was "very high" by 42.5% (17 elderly), "high" by 57.5% (23 elderly), "low" and "very low" by 0% (0 elderly). Conclusion: The motivation of elderly to participate Covid-19 vaccination in Ngijorejo Gari Wonosari Gunungkidul was high. Suggestion: For the government of Gari Wonosari Gunungkidul, this study is expected to educate the community about the importance of Covid-19 vaccination.
